What is Form I-693?

Form I-693, known as the Report of Immigration Medical Examination and Vaccination Record, acts as official document proof for individuals looking to apply for a Green card. The USCIS accepts Form I-693 as part of the evaluation for candidates migrating to the country. Individuals with this form can enter the country and qualify to apply for a Green Card.

 

Eligibility Criteria to file for Form I-693

The eligibility criteria given below must be fulfilled for Form I-693:

 

  • An authorized civil surgeon must examine the applicant.
  • The applicant must meet the regulations set forth by the U.S. Department of Health and Human Services (HHS).
  • Form I-693 must be fully completed and signed by the civil surgeon and the applicant.
  • The Form I-693 must have the signature of the authorized civil surgeon within two years of applying.
  • Form I-693 must have proof of information that the applicant has received the required vaccinations.
  • Form I-693 must prove that the primary applicant does not have a medical condition belonging to Class A.
  • The USCIS must decide on the form within two years of the signature from a civil surgeon.

 

Latest updates on the Form I-693

The USCIS recently announced a few updates to Form I-693. Forms signed by an official surgeon on or after November 1, 2023, will not expire and can be used as evidence to prove that the applicant is not suffering from any disease that can make them inadmissible. The latest policy will also apply to applications submitted before November 2023.
